1. Review the approved disability conditions as detailed on the Kansas Department of Revenue Division of Vehicles website to determine whether your disability qualifies and can be certified by an authorized health professional.
2. Download and print out the application "Form TR-159" from the Kansas Division of Vehicles website. Complete the top part of the application form including the selection of disability placards and/or disability plates.
4. Take the completed application to your County Treasurer's Motor Vehicle Office. In addition to the disabled placards and plates, a personal disabled identification card will also be issued.
3. Give the application form to your physician or other Healing Arts Licensed Professional so that the certification at the bottom part of the form can be completed. If the disability is temporary, the start and expected ending dates must be shown. If the disability is expected to exceed six months, it is considered to be a permanent disability.
