9. Sign and date your application. Provide your daytime telephone number.
10. Total your fees and make a check payable to Motor Vehicle Division.
11. Mail your application and fees to Mail Drop 8012, Special Plates Unit, Motor Vehicle Division, PO Box 2100, Phoenix, Arizona, 85001-2100.
12. Receive your specialty license plates within four weeks. Use your plates upon receipt.
